Our Dental Savings Plan covers essential services and offers discounts on a wide range of treatments:
Diagnostics & X-Rays
Periodic Exams (2/year) – 100% Covered
Bitewings (1 set/year) – 100% Covered
Full Mouth Series (FMX) or Panorex (1 every 5 years) – 50% Covered
Preventive Care
Adult Cleanings (2/year) – 100% Covered
Additional Cleanings – 10% Off

Our Dental Savings Plan is a savings program, not dental insurance. It’s designed to provide affordable access to dental care but comes with a few important limitations:
Cannot be used with dental insurance. This plan cannot be combined with any other dental coverage, discount plans, or third-party financing programs.
Specialist care and outside referrals are not included under this plan.
Hospital charges and care outside our office are not covered.
Services for injuries covered by worker’s compensation or medical insurance are excluded.
Products and retail items such as toothbrushes, whitening kits, and other non-treatment purchases are not discounted.
Invisalign and other orthodontic services are not included in the plan.
This plan is only honored at Mark Streitz Dental and cannot be transferred to another office or provider.
To help you get the most out of your Dental Savings Plan at Mark Streitz Dental, please review the following guidelines:
Non-Refundable: Membership fees are non-refundable and non-transferable, regardless of usage or circumstance.
Cannot Be Combined: This plan may not be used with any other dental insurance, discount plan, or promotional offer.
No Retroactive Discounts: Discounts apply only to services provided while the plan is active. Past procedures do not qualify.
Annual Renewal Required: The plan is valid for 12 months from the date of activation and must be renewed annually to maintain benefits.
Full Payment Required at Time of Service: All services must be paid in full on the day they are performed to receive the applicable discount.
No Additional Discounts: Other discounts or promotions cannot be used in conjunction with this plan.
